participates in most insurance plans and programs, including, but
not limited to, the following:
- Blue Cross/Blue Shield of Florida
- Cigna Healthcare
- Aetna U.S. Healthcare
- United Healthcare
- Beech Street
- Medicare
- Volusia Health Network
We will bill the above companies or plans directly for all
services provided. If you participate in an insurance plan that
requires prior authorization, please obtain the necessary
authorization before your visit.
You are responsible for all deductibles and co-payments at the
time of service; all office visits, for example, must be paid when
they occur. We accept Visa, MasterCard, and Discover for payment.
If you do not have insurance or are covered under an insurance
plan for which we are not a provider, please contact our office to
make payment arrangements.
